Our Mission

Patient-Centered Nursing Care with optimal patient outcomes.

The Transitional Care Unit is a newly opened 24-bed unit as of that accommodates SNF/ICF patients. The patient population that is cared for here on this unit are patients who are medically stable awaiting for a safe discharge plan. Our unit focuses on meeting patient-specific discharge goals such as working with Physical Therapy, Occupational Therapy, and working closely with the Social Workers and Case Managers. Our unit is smaller in size compared to the other Acute units, so the Nurses and Nurse Aides work very closely together, and we truly believe in teamwork to best serve our patients here.
