Our Mission

Patient-Centered Nursing Care with optimal patient outcomes.

The Transitional Care Unit is a newly opened 24-bed unit as of that accommodates SNF/ICF patients. The patient population that is cared for here on this unit are patients who are medically stable awaiting for a safe discharge plan. Our unit focuses on meeting patient-specific discharge goals such as working with Physical Therapy, Occupational Therapy, and working closely with the Social Workers and Case Managers. Our unit is smaller in size compared to the other Acute units, so the Nurses and Nurse Aides work very closely together, and we truly believe in teamwork to best serve our patients here.

What to Expect

The Transitional Care Nursing staff will greet you at the bedside when you arrive on the unit. You will be assigned to a specific Registered Nurse (RN) and a Certified Nurse Assistant (CNA). Your RN/CNA will immediately help you to get settled in your room and begin by introducing themselves and serving your needs. The staff will begin by admitting you to the hospital.  During this time, you will take part by answering important questions concerning your condition.  It does take some time to complete, but it is vital to making sure we have the most current and pertinent information necessary to provide the best care that we can for you.  After this, the RN/CNA will provide care based on your Physician ordered treatment plan. Remember, you (the patient) are an important member of the healthcare team.  Please feel free to ask questions and actively participate in your medical care.  We encourage it.

 

Bedside Shift Report: Our nurses will do their nursing report at your bedside to ensure that you are included in your plan of care.

Leader Rounding: During your time on our unit, a leader will round on you each day to ensure that exceptional care is being provided.

Hourly Rounding: Our staff will provide hourly rounding while you are on our unit. During this rounding, we will focus on pain control, position, getting you to the bathroom, and making sure that your personal possessions are within reach.

  • Nursing staff will perform hourly rounding to ensure all of your needs are met.
    • Hourly rounding will be done: every hour from 0600-2200 & every 2 hours from 2200-0600
  • Daily routine for patients:
    • Working with the necessary therapy teams for your strength and conditioning (physical therapy, occupational therapy, speech therapy)
    • Actively participate in your discharge planning
    • Actively participate in your activities of daily living
    • Mobilizing out of bed at least 3 times per day (up to chair, walking in the hallway, etc.)
  • TCU’s Quite Hours: 2200-0600.
    • Lights are required to be off during this time
    • If you would like to watch TV during these hours, you will be asked to use earphones which can be provided to you
    • Please use soft voices during quiet hours and be considerate to your roommates
